Case Mangement
Sojourner House employs a full-time Case Manager who provides case management to the families. The Case Manager works closely with the families making referrals to community resources based on the needs of each family in an effort to break down any barries or challenges that may arise.

Housing Search
Once a family moves into the shelter, their extensive search for affordable permanent housing begins. Each family begins to work on establishing credit, securing employment, money management, references, and various documents required for housing. Working with the Housing Advocate meets with each family onsite and works in conjunction with the Case Manager to secure permanent housing.

Stabilization
When a family moves out of the shelter to permanent housing they are provided Stabilization Services. The Housing Advocate continues to work with the family for two years ensuring that the family has a seamless transition into their new home and community. Monthly check-ins are conducted by the Housing Advocate.

Community Partners
We partner with local churches and organizations to offer families services such as onsite therapy, parenting classes, and community outings.
